Mr Abhi Gulihar Leading Shoulder, Elbow, Hand & Wrist Surgeon in the UK
Mr Abhi (Abhinav) Gulihar is a highly respected orthopaedic surgeon with recognised expertise in upper limb surgery, specialising in conditions and injuries of the shoulder, elbow, wrist, and hand. Based in London and Kent, he combines world-class surgical skill with a patient-centred approach to deliver comprehensive care tailored to individual needs.
With extensive training and fellowship experience both in the UK and internationally, Mr Gulihar is known for his proficiency in both non-surgical and surgical interventions—prioritising minimally invasive procedures where appropriate to support quicker recovery and enhanced outcomes for patients.
Early Training and Professional Development
Mr Gulihar’s academic journey laid the foundation for his excellence in orthopaedic surgery. He completed his specialist orthopaedic training in the East Midlands and Kent, Surrey & Sussex regions of the UK, before pursuing advanced fellowships in shoulder and elbow surgery at the South West London Elective Orthopaedic Centre and through the European Arthroscopy Fellowship. To further broaden his expertise, he undertook observational visits with leading shoulder surgeons in both Europe and the United States.
Mr Gulihar’s deep commitment to academic advancement is reflected in his postgraduate achievements, including a Masters in Orthopaedic Engineering and an MD focused on joint cartilage research at the University of Leicester. He continues to contribute actively to surgical education and research, with multiple peer-reviewed publications and presentations at international conferences.
Clinical Philosophy: Patient-Centred, Evidence-Based Care
Central to Mr Gulihar’s practice is his patient-centred philosophy. He believes that most upper limb conditions can be managed with conservative, non-surgical approaches, emphasising personalised care plans that consider each patient’s unique circumstances.
Where surgery is indicated, he adopts the latest minimally invasive arthroscopic (key-hole) techniques, which are designed to reduce postoperative discomfort and promote faster rehabilitation. This balance between conservative management and surgical innovation ensures patients receive optimal care tailored to their needs.
Specialist Areas of Expertise
Shoulder Conditions and Surgery
Shoulder problems can significantly impact daily life, affecting routine movements such as reaching, lifting, or driving. Mr Gulihar treats a wide range of shoulder conditions, including:
- Rotator cuff tears
- Frozen shoulder
- Shoulder instability and dislocations
- Shoulder arthritis
- Fractures and tendon tears
- Calcific tendonitis and acromioclavicular joint issues
Where necessary, he performs advanced surgical interventions—such as arthroscopic repairs, stabilisation procedures, and joint replacement—using techniques designed to preserve function and reduce recovery time.
Elbow Conditions and Management
Elbow injuries and degenerative conditions can hinder everyday tasks and athletic performance. Mr Gulihar addresses various elbow problems, including:
- Tennis elbow (lateral epicondylitis)
- Golfer’s elbow (medial epicondylitis)
- Arthritis and bursitis of the elbow
- Stiffness and loose bodies
- Ulnar neuropathy
He provides both surgical and non-surgical treatment plans, with options for arthroscopic procedures where appropriate to accelerate healing and restore elbow function.
Hand and Wrist Conditions
Pain and dysfunction in the hand and wrist can have profound effects on quality of life. Mr Gulihar treats a comprehensive spectrum of hand and wrist injuries and conditions, including:
- Carpal tunnel syndrome
- Trigger finger and thumb
- Base of thumb arthritis and Dupuytren’s disease
- Ganglions, cysts and common lumps
- Fractures of the wrist and metacarpals
His surgical approach emphasises precision and functional preservation, ensuring patients return to normal activity with minimal disruption.
Sports Injuries and Advanced Procedures
Upper limb injuries are common in sports such as cricket and golf, where repetitive motions and high forces can lead to tendon tears, ligament injuries, or joint breakdown. Mr Gulihar has substantial experience managing sports-related shoulder, elbow, and wrist injuries, helping athletes achieve optimal outcomes through both conservative care and advanced surgical repair.
He is also highly skilled in regenerative treatments like Platelet-Rich Plasma (PRP) injections, which can relieve pain and accelerate recovery in cases of osteoarthritis and chronic tendon injuries—often deferring or reducing the need for surgical intervention.
The Patient Journey: From Consultation to Recovery
1. Initial Assessment
Each patient begins with a detailed consultation, where Mr Gulihar listens carefully to symptoms, reviews diagnostic imaging where necessary, and discusses all available treatment options.
2. Personalised Treatment Plan
Following assessment, an evidence-based care plan is crafted to suit the individual’s goals—whether focusing on non-surgical therapies or recommending surgery if clearly indicated.
3. Surgery and Rehabilitation
When surgery is performed, advanced techniques such as key-hole surgery or joint replacement help reduce recovery time and facilitate effective rehabilitation. Throughout recovery, Mr Gulihar and his team provide ongoing support to ensure the best possible outcomes.
Professional Recognition and Patient Feedback
Across his career, Mr Gulihar has earned high praise for his clinical expertise, thorough communication, and compassionate care. Patients frequently comment on his clear explanations, attentive follow-up, and ability to deliver successful surgical outcomes that restore function and improve quality of life.
His leadership in orthopaedic education and research has also been recognised with awards and invitations to teach at national and international surgical courses.
