NEJM Oct 22 2026 Response-Tailored or Standard-Duration Antibiotic Treatment for Infective Endocarditis

1m | Oct 28, 2026

A recent international clinical trial investigated whether shortening antibiotic regimens for left-sided infective endocarditis is safe and effective once patients achieve clinical stability. Researchers compared a response-tailored strategy—which stopped antibiotics earlier—against the standard six-week duration across more than 500 participants. The findings revealed that the tailored approachsuccessfully increased the number of days patients spent free of medication without increasing major safety risks like mortality or surgery. However, this shorter treatment method carried a trade-off, resulting in a statistically higher rate of infection relapse compared to the conventional timeline. Ultimately, the study demonstrates that while individualized antibiotic duration offers greater freedom from drugs safely, clinicians must carefully weigh this benefit against the increased danger of recurrence.
